Job summary

Cynet Health seeks an experienced Registered Nurse for pediatric patients to support the Pediatric Complex Care Program. The role is hybrid, with up to 3 in-person days and 2 remote days, five 8-hour days per week, no weekends, nights, on-call, or holidays.

Responsibilities include coordinating care, patient education, EPIC in-basket management, and communication with families, DME, and vendors. Medication reconciliation is not part of this role.

Qualifications

• Active RN license.
• Strong current pediatric experience.
• BLS certification.

Responsibilities

• Coordinate care and clinically manage patients and caregivers.
• Patient education and reviewing after-visit summaries.
• Manage EPIC in-basket, triage calls, and return messages to families.
• Coordinate with DME, nursing agencies, and other vendors.
• Review patient charts as needed.
