Job summary

Angels of Care Pediatric Home Health in Texas seeks a Skilled Visit Coordinator to organize timely, accurate scheduling of skilled clinical visits across branch offices. You will serve as a crucial link among clients, families, field clinicians, clinical leadership, and support teams to ensure continuity of care and efficient service delivery.

The role requires meticulous planning, proactive problem-solving, HIPAA compliance, and the ability to escalate concerns to leaders.

Qualifications

• 3+ years of healthcare coordination or scheduling experience.
• Experience in home health or pediatric care preferred.
• Experience with EMR/scheduling systems preferred.

Responsibilities

• Coordinate skilled clinical visits to support timely delivery of ordered and authorized services while promoting continuity of care for clients and families.
• Maintain accurate clinician schedules, visit assignments, and related information within scheduling, EMR, and operational systems.
• Partner with clinical leadership and field clinicians to align visit assignments with client needs, clinician availability, qualifications, geographic considerations, and organizational requirements.
• Monitor upcoming and outstanding visits and proactively address scheduling gaps, cancellations, conflicts, or other barriers that could disrupt continuity of care.
• Communicate schedule changes and visit-related information promptly and professionally to clinicians, clients, families, and internal stakeholders.
• Identify scheduling opportunities, balance priorities, and help minimize missed or delayed visits.
