Job summary

Angels of Care Pediatric Home Health seeks a Skilled Visit Coordinator to support timely scheduling and coordination of skilled clinical visits from any Texas branch office. Occasional travel may be required as needed to ensure continuity of care and regulatory compliance.

The role emphasizes organization and strong communication to align client needs with clinician availability, while maintaining accurate records and HIPAA-compliant practices.

Qualifications

• Must be a licensed RN.
• 3+ years of healthcare coordination or scheduling experience.
• Experience in home health or pediatric care preferred.
• Experience with EMR/scheduling systems preferred.
• Strong organizational and communication skills.

Responsibilities

• Coordinate skilled clinical visits to ensure timely services and continuity of care.
• Maintain accurate clinician schedules, visit data, and related information.
• Collaborate with leadership and clinicians to align visits with client needs.
• Monitor upcoming visits and address scheduling gaps to avoid disruptions.
• Communicate schedule changes promptly to clinicians, clients, families, and stakeholders.
• Support effective clinician utilization by identifying scheduling opportunities and balancing priorities.
• Maintain documentation for scheduling activity per standards.
• Identify and escalate unresolved scheduling or service-delivery concerns to leadership.
• Collaborate across departments to enable coordinated client service and care transitions.
• Oversee training and competencies for all skilled visits nurses.
• Monitor workflows to ensure timely completion of actions.
• Protect confidential information per HIPAA and policies.
• Contribute to process improvement and service quality initiatives.
