Angels of Care Pediatric Home Health is seeking a Skilled Visit Coordinator to support timely delivery of skilled clinical visits for pediatric patients. The role acts as a crucial link among clients, families, field clinicians, and internal support teams to ensure continuity of care and compliant service delivery.

Qualifications

• Registered Nurse is required.
• 3+ years of experience in healthcare coordination, scheduling, operations, customer service, or a comparable environment.

Responsibilities

• Coordinate skilled clinical visits to support timely delivery of ordered and authorized services.
• Maintain accurate clinician schedules, visit assignments, and related information within scheduling, EMR, and operations systems.
• Collaborate with clinical leadership and field clinicians to align visit assignments with client needs and clinician availability.
• Monitor upcoming visits and address scheduling gaps or conflicts to prevent disruption of care.
• Communicate schedule changes promptly to clinicians, clients, families, and internal stakeholders.
• Support effective clinician utilization by identifying scheduling opportunities and balancing priorities.
• Maintain documentation related to scheduling activity and follow-up in accordance with standards.
• Escalate unresolved scheduling or service-delivery concerns to leaders for timely resolution and compliance.
• Collaborate across departments to facilitate coordinated client service and care transitions.
• Oversee training and competencies for all skilled visits nurses.
• Monitor workflows to ensure actions are completed within timelines.
• Protect confidential information per HIPAA and policies.
• Contribute to process improvement and department initiatives.
