Job summary

Angels of Care Pediatric Home Health is seeking a Skilled Visit Coordinator to support the timely coordination of skilled clinical visits across Texas branches. The role acts as a key operational link among clients, families, field clinicians, and support teams to promote continuity of care and compliance.

You will maintain accurate schedules, address barriers, and escalate concerns promptly to ensure safe, timely delivery of authorized services with HALOS values guiding daily work.

Qualifications

• RN license required.
• 3+ years of healthcare coordination, scheduling, or operations experience.

Responsibilities

• Coordinate skilled clinical visits to ensure timely delivery of services.
• Maintain accurate clinician schedules and visit information.
• Partner with leadership and field clinicians to align visits with client needs and resources.
• Monitor upcoming visits and address scheduling gaps or conflicts.
• Communicate schedule changes promptly to clinicians, clients, families, and internal stakeholders.
• Support clinician utilization by balancing priorities and minimizing missed visits.
• Document scheduling activity and follow-up in systems per standards.
• Escalate unresolved scheduling or service-delivery concerns to leadership.
• Collaborate across departments to enable coordinated client service and smooth care transitions.
• Oversee training and competencies for skilled visits nurses.
• Monitor workflows to ensure timely actions are completed.
• Protect HIPAA and privacy compliance in all duties.
• Contribute to process improvement and team coverage initiatives.
