Southcentral Foundation (SCF) invites applications for an RN Case Manager in Anchorage, AK. This role focuses on delivering timely, patient-centered care for customer-owners, aligning with SCF standards and Advanced Access principles to improve health outcomes and cost-effectiveness.

Requirements include graduation from an accredited nursing program, Alaska RN license, BLS (ACLS/NRP/ATLS/ALSO as required), and at least one year of nursing experience or completion of the SCF RN Case Manager

Qualifications

• Graduate of an accredited school of nursing required.
• Alaska RN license required.
• BLS certification is required. ACLS, NRP, ATLS, ALSO and PALS as required for specialty.
• One year nursing experience or completion of the SCF RN Case Manager Training Program.

Responsibilities

• Deliver appropriate, timely, and beneficial care for customer-owners within established case management standards and Advance Access principles.

Education
