• At least 5 years of clinical experience in pediatric hematology/oncology
• NJ State Professional Registered Nurse License
• CPHON certification preferred (must be certified within one year if not currently certified)

Responsibilities

• Collaborate with physicians, nursing staff, and interdisciplinary team in patient care.
• Identify problems during initial assessments and help adhere to evidence-based guidelines.
• Maintain a communication system for timely information transmission to the healthcare team.
• Support immediate revision of the plan of care and initiate patient education.
• Monitor documentation in the medical record to ensure patient outcomes.
• Identify and link patients to community partners and services.
• Participate in continuous process improvement efforts to enhance patient outcomes.
• Modify and deliver care specific to patient population needs.
• Communicate effectively with patients and caregivers at their level.
• Attend unit, departmental, and hospital meetings as needed.]
