Job summary

Hackensack Meridian Health seeks a Patient Navigator for Pediatric Hematology and Oncology to coordinate care across physicians, nurses, and the care team. You will support patients from initial consultation through treatment, ensuring adherence to guidelines and providing education to patients and families.

The role requires an RN with at least 5 years in pediatric hematology/oncology and a valid NJ nursing license.

Qualifications

• RN with at least 5 years of clinical experience in pediatric hematology/oncology.
• NJ State Professional Registered Nurse License.
• Excellent communication skills.
• Strong computer skills.
• Proficient with Microsoft Office and Google Suite.
• Ability to work independently and collaborate with the healthcare team.

Responsibilities

• Participates in collaboration with physicians, nursing staff, and interdisciplinary team in assessment, planning, implementation and evaluation of care for selected patients and their families.
• Identifies problems during initial assessments and supports adherence to guidelines to meet outcomes.
• Maintains a communication system for timely information transmission to physicians and team.
• Initiates education with the oncology patient and family to support outcomes.
• Monitors documentation to ensure outcomes are achieved per standards of care.
• Links patients to available community partners and services.
• Participates in process improvement to enhance patient outcomes and satisfaction.
• Provides compassionate care and teamwork with colleagues.
• Adapts care to patient needs (age, culture, language, accessibility) and communicates accordingly.
• Attends unit/department meetings and committees as needed.
