Job summary

Tri-Valley, Inc. is seeking a Registered Nurse to participate in screening and ongoing monitoring of PCA program participants in homes and other settings in Massachusetts.

The role requires a MA RN license, strong communication skills, and experience with PCA programs and MassHealth processes. The RN will collaborate with therapists, maintain documentation, and support outreach within the community.

Qualifications

• Valid MA RN license is required.
• Regular and reliable attendance is expected.
• Excellent organizational and time management skills.
• Strong communication with consumers, families and providers.
• Confidentiality and professional conduct must be maintained.
• Proficiency with standard office software and healthcare systems.

Responsibilities

• Screen program participants in homes or other settings per PCM program rules.
• Assess physical and cognitive function to determine PCA services needed.
• Conduct face-to-face interviews to finalize service requests.
• Obtain MD signature on evaluations and document approvals.
• Input evaluation data into the PCA software system.
• Coordinate with Program Director, FST and PCA consultants.
• Provide education to PCA staff on personal health care vs hands-on care.
• Assist in outreach to grow the PCA program in communities served.
