Zusammenfassung

Activate Care is seeking a field-based Registered Nurse in Massachusetts to perform in-person health assessments for dually eligible members. You will travel to homes and community settings, document findings, and hand off to care management.

This role emphasizes assessment and documentation rather than ongoing case management. The ideal candidate holds an MA RN license, an ADN (BSN preferred), and at least 1 year of home health and assessment experience.

Qualifikationen

• Active Massachusetts RN license (or higher) in good standing
• Accredited, Associate Degree in Nursing required; BSN preferred
• Valid driver's license, reliable personal vehicle, and active auto insurance required
• 1+ years of home health clinical experience
• 1+ years of assessment experience preferred (RAC-CT or similar certification a plus)
• Experience with dual-eligible, Medicaid, or Medicare Managed Care populations preferred

Aufgaben

• Conduct In-Person Assessments in the Field: Routinely travel to members' homes and community settings to perform required assessments evaluating physical, cognitive, and functional status
• Comprehensive Care Management Documentation: Document assessment findings accurately in the health plan's care management platform following established workflows
• Maintain High Quality & Timely Submissions: Submit complete documentation within 48 hours of each assessment, meeting standards
• Uphold Safety & Mandated Reporting Standards: Follow field-safety guidelines and adhere to mandated reporting laws
• Ensure Regulatory & Compliance Alignment: Adhere to HIPAA, NCQA, and regulatory requirements while completing training
• Other duties as assigned
