Job summary

Fallon Health in Massachusetts is seeking an Assessment Nurse Case Manager to perform in-home onboarding and regulatory assessments for new enrollees, including Health Risk Assessments and PCA evaluations. Most visits are in person, with occasional telephonic assessments, working with the Care Team to submit MDS data timely.

The role requires a registered nurse license in Massachusetts, strong communication and care coordination skills, and reliable transportation for home visits; OASIS/MDS

Qualifications

• Graduate from an accredited school of nursing and MA RN licensed.
• 1+ year of clinical RN experience with complex needs preferred.
• Strong communication and care coordination skills.

Responsibilities

• Conducts in-home onboarding and regulatory assessments.
• Completes HRAs and PCA assessments.
• Submits MDS assessments in a timely manner.
• Educates members on NaviCare case management.
• Performs telephonic assessments when appropriate.
• Collaborates with the interdisciplinary care team.
• Completes LTSS evaluations with UM collaboration.
