Qualifications

• Bachelor's degree in nursing or related field required.
• Current RN license in the state of practice; BLS certification required.
• 5 years of clinical experience; strong organizational and care coordination skills.
• Excellent communication, writing, time management; familiarity with word processing and databases.
• Ability to teach clinical and non-clinical personnel; process improvement experience preferred.

Responsibilities

• Assesses the patient’s plan of care and coordinates resources and progress.
• Evaluates medical necessity and coordinates with physicians; schedules initial tests and consultations.
• Assists with diagnostic services, treatment and care; coordinates disease care with an interdisciplinary team.
• Ensures patient and family understand diagnostic processes, care strategy and actions; provides information.
• Sets functional, measurable goals; develops discharge plan and coordinates referrals.
• Supports patient/family through difficult decisions and end-of-life planning; provides emotional support.
• Contributes to process-improvement activities for patient populations and outcomes.
• Establishes collaboration with physicians, payers, and health team; communicates with payers for requirements.
• Works independently within established procedures; seeks supervisor guidance for unusual situations.
