Qualifications

• Bachelor's degree in nursing or related field.
• Current RN license in the state.
• BLS certification.
• 5 years of clinical experience.
• Strong organizational and care coordination knowledge.
• Effective communication and writing skills.
• Ability to teach clinical and non-clinical personnel.
• Understanding of process improvement.

Responsibilities

• Assess plan of care and coordinate resources across the continuum.
• Evaluate medical necessity and coordinate initial tests and referrals.
• Assist with diagnostic services and referrals in coordination with physicians.
• Explain care plans and provide information to patients and families.
• Support end-of-life care discussions and discharge planning.
