You will work with the clinical care team to ensure timely diagnoses and treatments, identify barriers to care, and connect patients with resources to maintain health and well-being throughout treatment.

Qualifications

• High school diploma or equivalent required; medical terminology knowledge.
• Experience in healthcare or patient support roles preferred.
• Ability to verify insurance and manage documentation.

Responsabilités

• Coordinate resources for patients with identified barriers to care.
• Gather data, complete documentation, and escalate issues when needed.
• Use Banner’s phone system to communicate with patients and providers, HIPAA compliant.
• Review medical records or test results to plan next steps in care coordination.
• Coordinate with physicians to optimize outcomes and align for consultations.
• Manage multiple appointments using scheduling systems.
• Work independently within established procedures to support patient care coordination.
