Job summary

Banner Health Corporate in Phoenix, AZ is seeking an RN Patient Navigator to coordinate care for patients with addiction and behavioral health needs. The role involves education, symptom management, and care plan adherence across inpatient, outpatient, and community settings, including psychiatry collaboration and specialty consults.

The position is full-time, 40 hours/week, day shift, with a schedule from 8:00 to 16:00 Monday through Friday, with flexibility for patient needs.

Qualifications

• Must possess knowledge as normally obtained through the completion of a Bachelor’s degree in nursing or related field.
• Requires a current RN license in the state of practice.
• BLS certification required.
• Requires 5 years clinical experience.

Responsibilities

• Assesses and coordinates patient care using an interdisciplinary approach across inpatient, outpatient and community settings.
• Educates patients and families, manages symptoms and ensures adherence to the care plan.
• Coordinates tests, consultations and referrals with physicians and providers.
• Supports discharge planning and ongoing care coordination to reduce readmissions.
