Job summary

Baylor Scott & White Health's Education Department in Fort Worth is seeking a Patient Navigator RN. The role focuses on coordinating patient care, assessing needs, and collaborating with the medical team to plan, implement, and evaluate care.

You will work autonomously and as part of a team, advocating for patients and guiding them through the hospital. The ideal candidate has a minimum of 2 years of maternal care RN experience, is licensed as a Registered Nurse, and can obtain Basic Life

Qualifications

• Minimum 2 years of maternal care RN experience.
• Registered Nurse license required.
• Willing to obtain Basic Life Support (BLS) within 30 days.

Responsibilities

• Evaluate health and create a care plan based on doctor's orders.
• Schedule outpatient tests and coordinate with the care team.
• Assist patients in navigating the hospital and provide information.
• Maintain nursing records and update the Navigator database.
• Attend case conferences and participate in community service.
• Support care coordination and improve patient experience.
