• Evaluate patient and family needs across healthcare, education, and psychosocial areas.
• Educate patients, visit, and communicate to support adherence to care plans.
• Coordinate resources to fulfill medical and non-medical patient needs.
• Collaborate with programs to ensure proper service coordination.
• Identify issues and propose care coordination improvements.
• Communicate with hospital, long-term care, and home health representatives.
