Job summary

Advocate Health in Wake Forest area is seeking an RN Navigator program coordinator to lead quality improvement and care coordination. You will ensure high-quality patient care, engage families, and collaborate with community partners to meet patient needs across the care continuum.

The role requires RN licensure, BSN is preferred with relevant case management experience; Spanish language skills are a plus. This full-time, on-site position offers comprehensive benefits and professional growth

Qualifications

• Graduation from an accredited School of Nursing. BSN preferred with two years' experience as a Registered Nurse.
• Case management experience preferred.
• Bilingual preference with fluency in Spanish preferred.

Responsibilities

• Ongoing evaluation and quality improvement of the Navigator Program and tracks all activity as defined by the departmental and program standards.
• Ensures quality patient care by adhering to policies, engages patients and families to identify needs and plans across the continuum of care.
• Implements outreach to establish and maintain relationships with key community agencies.
• Collects and maintains up-to-date program data; delivers reports showing outcomes and performance improvements.
• Trained in motivational interviewing and health coaching to provide chronic care management.
• Participates in professional development and seeks feedback for continuous improvement.
• Provides age/developmentally appropriate patient care according to age guidelines.
