Job summary

Advocate Health seeks a Registered Nurse Navigator for the Wake Forest Baptist Medical Center – Clinic: Pediatric. This full-time role oversees program evaluation, quality improvement, and patient-centered care coordination with families across the continuum.

The ideal candidate holds RN licensure with BSN preferred and two years of RN experience, plus case management and bilingual Spanish as a plus. MD/clinic team collaboration and ongoing professional development are emphasized.

Qualifications

• Graduation from an accredited School of Nursing.
• BSN preferred with two years' experience as a Registered Nurse.
• Case management experience preferred.
• Bilingual Spanish preferred.

Responsibilities

• Ongoing evaluation and quality improvement of the Navigator Program and tracking of activity per standards.
• Ensure quality patient care by following policies and coordinating with patients and families across the care continuum.
• Build outreach efforts to establish positive relationships with community agencies.
• Collect and maintain program data and documentation; deliver reports showing outcomes and performance improvement.
• Trained in motivational interviewing and health coaching for chronic care management.
• Participate in shared governance and ongoing professional development.
• Provide age-appropriate patient care per age-specific guidelines.
