Job summary

Advocate Health in Charlotte, NC is seeking a dedicated RN to fill the Patient Navigation role in Morehead. You will educate patients and families, coordinate referrals, and serve as a liaison across departments to ensure timely access to care.

Requires BSN, RN licensure in NC (MSN preferred), strong communication, and ability to arrange resources for insurance, transportation, and follow-up. Full-time, 40 hours weekly, days 8am-4:30pm with competitive benefits.

Qualifications

• BSN required; MSN preferred.
• Current RN licensure in North Carolina.
• Certification in the appropriate field within 2 years.
• Excellent communication skills required.

Responsibilities

• Provides language specific education and psychosocial support for newly diagnosed patients and families.
• Educates patients on diagnosis, work-up, treatments, and follow-up options.
• Acts as liaison for patients and families between departments.
• Coordinates referrals and scheduling of exams and appointments.
• Contacts every newly diagnosed patient at the time of diagnosis.
• Assesses emotional and social needs and refers to resources.
• Identifies barriers to care and participates in service recovery.
