Job summary

You will educate patients and families on diagnosis, work-up, treatments including surgery and supportive care, and collaborate with multidisciplinary teams to address barriers such as insurance and transportation while ensuring timely follow-up.

Qualifications

• BSN degree required; MSN preferred.
• Current licensure as a Registered Nurse in the practicing state.
• Excellent communication skills required.

Responsibilities

• Educates patients and families about diagnosis, treatments, and support services.
• Coordinates referrals and liaises between departments to ensure seamless care.
• Identifies barriers to care (insurance, transportation) and connects resources.
• Contacts newly diagnosed patients to assess emotional and social needs.
• Supports patient satisfaction and coordinates care for follow-up.
• Addresses psychosocial needs and provides language-specific education as needed.
• Ensures timely scheduling of exams, procedures, and appointments.
