Job summary

Ascension St. Vincent in Indianapolis, IN seeks a Patient Navigator in Hematology/Oncology to guide patients through the continuum of care. This full-time, day schedule supports coordination with physicians, management, and care teams to ensure seamless treatment pathways.

The role emphasizes data tracking, outcome monitoring, and process improvements, with referrals and follow-up support for underinsured or uninsured patients. RN licensure in Indiana is required, with BLS preferred.

Qualifications

• Registered Nurse credentialed from the Indiana Board of Nursing obtained prior to hire date or job transfer date required.
• BLS Provider preferred. American Heart Association or American Red Cross accepted.

Responsibilities

• Serve as a patient navigator throughout the continuum of care.
• Function as a liaison for management staff, administration, physicians, managed care companies, community organizations, and other customers.
• Coordinate efforts for treatment and promote quality improvement and educational efforts.
• Compile data, tracks outcomes, and make recommendations for process improvement.
• Provide referrals/support for follow-up care for patients who are uninsured or underinsured.

What Minimum Requirements You’ll Need

Licensure / Certification / Registration:

• Registered Nurse credentialed from the Indiana Board of Nursing obtained prior to hire date or job transfer date required.
• BLS Provider preferred. American Heart Association or American Red Cross accepted.

Education

• Diploma from an accredited school/college of nursing OR required professional licensure at time of hire.

What Additional Preferences We're Seeking

• Oncology Certified Nurse (OCN) preferred
• Two years of experience in Oncology and or Case Management Preferred
