You will collaborate with the healthcare team to plan, implement, and evaluate individualized care plans while engaging patients and families in decision making.

The role emphasizes safety, efficient discharge planning, and ongoing quality improvement. Completion of an accredited nursing program and an active RN license are required, with BLS certification
