Job summary

Geisinger in Danville, PA seeks an experienced Registered Nurse to provide patient education, coordinate pre- and post-hospital care, and ensure continuity of care for patients. The role is full-time, 40 hours weekly, days, with at least 2 years of RN experience required.

You will liaise between patients and caregivers, coordinate multidisciplinary clinics, and support providers and teams in delivering comprehensive care. Generous benefits and tuition assistance are offered.

Qualifications

• At least 2 years of RN work experience required.

Responsibilities

• Provide patient education and ongoing continuity of care.
• Coordinate pre- and post-hospital care; serve as liaison between patient and caregivers.
• Coordinate appropriate multidisciplinary clinics and guide incoming patients into the care program.
• Provide clinical support to the provider and patients for diagnosis and treatment.
• Collaborate with other healthcare providers to coordinate and implement the patient's plan of care.
• Communicate with patients via mail, telephone, and in person about information pertinent to their care.
• Reinforce patient and family education based on individual needs, in person or by telephone.
• Maintain systems to assure timely follow-up and surveillance of post-follow-up patients.
• Educate other team members in area of expertise.
