Responsibilities

• Will be a Licensed Practical Nurse performing duties as a patient navigator guiding incoming calls and facilitating entrance of new patients into the program.
• Provides patient education, promotes an understanding of the condition, and provides ongoing continuity of care for patients.
• Nurse Navigator collaborates with the provider and patients to assist in diagnosis and treatment, coordinating patient activities and scheduling as needed.
• Maintains systems and processes to assure timely follow-up and surveillance of patients.
