Qualifications

Responsibilities

• Provide preventative patient care as part of an interdisciplinary team.
• Coordinate disease management and screening services.
• Support the provider, patient, and family in diagnosis and treatment plans.
• Triage in-basket messages and manage patient interactions with the clinic.
• Administer medications and vaccines under provider supervision.
