Job summary

CenterWell is hiring a Patient Care Coordinator to support discharge planning and patient care coordination across hospitals and facilities. The role involves facility visits, collaboration with clinical teams, and processing physician orders to ensure program eligibility and service delivery.

The position requires nursing licensure (RN/LPN/LVN) and preferably a BSN, with at least 3 years of clinical experience and strong regulatory knowledge.

Qualifications

• Excellent customer service, time management, problem-solving and communication skills.
• Travel within an assigned territory and to sales meetings as required.
• 3 years clinical experience is preferred.
• Must have strong knowledge of governmental regulations, Medicare eligibility requirements, comprehensive understanding of potential care plan needs for the patient and coordination of necessary resources.
• RN/LPN/LVN license is required.
• Bachelor’s of Science in Nursing is preferred.
• This role is considered patient facing and is part of Human’s TB screening program. If selected for this role, you will be required to be screened for TB.

Responsibilities

• Develop and maintain contact with key hospital, skilled nursing, and facility discharge planning services to provide ongoing updates on CenterWell services.
• Primarily conduct facility visits at physicians’ request to assist program clinical team in determining eligibility.
• Collaborate with program staff and other field sales resources via weekly meetings; participate in weekly program business development meetings.
• Assist program in processing physician orders as directed.
• Report to a Market Executive of Sales.
