Job summary

The role requires an RN with a BSN, experience in medical/surgical, critical care or community health, and the ability to coordinate care, assess needs, and liaise with patients, families and external agencies to arrange equipment and services at discharge.

Qualifications

• Graduate RN, preferably with BSN.
• Medical/Surgical, Critical Care or Community Health nursing experience required.
• Knowledge of discharge planning processes and utilization review.

Responsibilities

• Implements discharge planning for a unit’s patient population.
• Assesses patients' medical, rehabilitation and functional needs and coordinates family support.
• Performs on-site admission and chart reviews to determine appropriateness of hospital stay.
• Maintains knowledge of Medicare/Medicaid and third-party coverage and resources.
• Initiates referrals and serves as liaison with home health and community services.
• Monitors progress and communicates needs to care teams and departments.
• Leads discharge planning rounds and advises on management of hospitalization.
• Documents progress and maintains records per policy.
