Job summary

CenterWell Home Health in Buffalo Grove, IL, seeks a Patient Care Coordinator to establish and maintain connections with hospitals, skilled nursing and discharge planning services, ensuring ongoing updates on services available in the market.

You will conduct facility visits at physicians' request to help determine eligibility and coordinate with program staff in weekly meetings. You will process physician orders and report to a Market Executive of Sales.

Qualifications

• RN/LPN/LVN license is required.
• Bachelor's of Science in Nursing is preferred.
• 3 years clinical experience is preferred.
• Must have strong knowledge of governmental regulations, Medicare eligibility requirements, and care coordination of patient needs.
• Excellent customer service, time management, problem-solving and communication skills.
• Travel within an assigned territory and to sales meetings as required.
• This role is part of Humana's TB screening program; if selected you will be screened for TB.

Responsibilities

• Develop/maintain contact with key hospitals, skilled nursing facilities and discharge planning services to provide updates on CenterWell Home Health services in the market.
• Primarily conduct facility visits at physicians' request to assist the program clinical team in determining eligibility.
• Collaborate with program staff and other field sales resources via weekly meetings and participate in weekly program business development meetings.
• Assist program in processing of physician orders as directed.
• Report to a Market Executive of Sales.
