Job summary

CenterWell Home Health is seeking a Patient Care Coordinator in Illinois to develop and maintain relationships with hospitals, SNFs and discharge planning services. You will conduct physician-requested facility visits to support eligibility determinations and collaborate with program staff in weekly meetings.

The role involves processing physician orders and reporting to a Market Executive of Sales. The position requires an RN/LPN/LVN license, with BSN preferred and about 3 years of clinical

Qualifications

• RN/LPN/LVN license is required.
• Excellent customer service, organization, time management, communication and selling skills.
• Travel within an assigned territory and to sales meetings as required.
• This role is patient facing and part of TB screening program.

Responsibilities

• Develop/maintain contact with key hospital, SNF, and discharge planning services to provide updates on CenterWell Home Health services.
• Primarily conduct facility visits at physicians' request to assist program clinical team in determining eligibility.
• Collaborate with program staff and other field sales resources via weekly meetings and participate in weekly program business development meetings.
• Assist program in processing physician orders as directed.
• Report to a Market Executive of Sales.
