Job summary

CenterWell is seeking a Patient Care Coordinator to remotely manage discharge planning communications with hospitals, skilled nursing facilities and assisted living centers, providing timely updates on services and care options.

You will support the clinical team in determining patient eligibility, conduct occasional facility visits at physician request, and collaborate with program staff in weekly meetings. Some travel to training or meetings may be required.

Responsibilities

• Develop/maintain contact with hospital, SNF, and assisted living discharge planning to provide updates on services.
• Primarily conduct facility visits at the physician's request to assist in determining eligibility.
• Ensure effective communication with program staff and field sales via weekly meetings.
• Assist the program in processing physician orders as directed.
• Report to a Market Executive of Sales.
