Fort HealthCare seeks an RN Patient Care Coordinator to manage safe and efficient discharge planning. The role collaborates with physicians, nursing staff, and community resources to develop post-acute care plans aligned with patient goals.

Shift: Part time, 0.8 FTE (64 hours per pay period); 8:00 a.m.–4:30 p.m.; Every 5th Saturday 8:00 a.m.–4:30 p.m. Fort HealthCare is committed to high-quality patient care and community health improvement.

Qualifications

• ADN required and BSN preferred.
• Two years of nursing experience required.
• Strong understanding of the Nursing Process and care planning.
• Excellent communication and organizational abilities with all disciplines.
• Ability to collect, interpret and act on data.

Responsibilities

• Coordinate care for patients meeting case management criteria through collaboration with team members.
• Engage patients and caregivers to assess self-care and align discharge goals.
• Coordinate care across the continuum to achieve quality, clinical and cost outcomes.
• Participate in daily multidisciplinary rounds to identify discharge needs and referrals.
• Facilitate communication among the healthcare team, patient, caregiver and external partners.
• Assist other departments with care coordination and discharge planning as needed.
• Perform admission, concurrent and discharge reviews for adherence to criteria.
