Job summary

Health, Inc is seeking a Patient Care Coordinator to oversee care management for inpatient populations, collaborating with physicians, nursing, social workers, and community providers to ensure smooth care transitions, timely discharges, and prevention of readmissions.

The role requires at least 1 year of acute care nursing, an ASN, and a multi-state RN license with BLS, plus EMR experience and strong communication, organizational, and critical thinking skills.

Qualifications

  • At least 1 year of acute care nursing experience.
  • ASN required for RN licensure.
  • RN license with multi-state credentials.
  • BLS certification.
  • Experience with EMR systems.
  • Strong communication, organizational, and critical-thinking skills.
  • Prior experience in acute care, case management, or home health preferred.
  • BSN advantageous.

Responsibilities

  • Conduct initial assessments within 24 hours of admission.
  • Document and communicate discharge plans.
  • Participate in daily rounds.
  • Coordinate post-acute care referrals.
  • Manage delays or barriers to discharge.
  • Ensure follow-up appointments are scheduled.
  • Maintain compliance with safety and regulatory policies.
  • Participate in quality improvement initiatives.
  • Foster effective communication with patients, families, and healthcare teams.
