Job summary

Qualifications

• Graduation from an accredited school of nursing meeting the minimum standards set by the State of Alabama.
• RN licensure in accordance with Alabama requirements or compact state licensure.

Responsibilities

• Communicates with physicians, patients, families and healthcare team in person, via email, via telephone, or other communication tools.
• Responds to overhead pages and coordinates discharge planning.
• Identifies patient needs and resources to formulate an appropriate discharge plan.
• Collaborates with healthcare team to schedule post-acute services and options.
• Documents assessments and maintains compliance with department guidelines.
