Job summary

Qualifications

Responsibilities

• Perform initial face-to-face assessment within 24 hours of admission to determine discharge needs.
• Document outcomes in the EMR and communicate transition plans with care team.
• Participate in daily rounds to track progress toward discharge.
• Coordinate with social work for long-term care and post-acute services.
• Schedule follow-up appointments and referrals prior to discharge.
• Maintain infection control and patient safety compliance.
• Collaborate with physicians, nurses, and family to ensure timely discharge and prevent readmission.
