Job summary

Kaiser Permanente is seeking a Patient Care Coordinator to oversee the management and coordination of care for the acute inpatient population. The PCC collaborates with physicians, patients and families, nursing staff, and other care team members to ensure cost-effective outcomes and safe discharge planning.

Qualifications

• Minimum of one year in an acute medical/surgical/ED or critical care nursing area.
• Experience using an electronic medical record system.
• Strong communication and problem-solving abilities.
• Ability to work independently and manage priorities in a high-volume environment.
• Excellent customer service and teamwork skills.

Responsibilities

• Complete an initial face-to-face assessment for admitted patients within 24 hours of admission.
• Document assessment outcomes to determine the transition plan with the care team.
• Participate in daily Care Without Delay rounds and report patient progression.
• Review and document discharge plan in accordance with policies and regulatory requirements.
• Escalate delays and identify systemic patterns requiring corrective action.
• Coordinate with social workers for long-term care, financial assistance, and other services.
• Send referrals to in-network vendors for post-acute levels of care.
• Ensure follow-up appointments and referrals are scheduled prior to discharge.
• Maintain safety policies and participate in Quality Assurance duties.
