This role acts as a liaison and communicator with patients, caregivers, healthcare providers, and the multidisciplinary team, including post-acute care and third-party payers. The nurse assesses and addresses the needs of patients with advanced disease or at the end of life, providing educational support to patients, families, and healthcare professionals, including physicians and fellow nurses, in a positive and empathetic manner.

Education
  • Required: Graduate of an accredited nursing school.
Work Experience
  • Preferred: Bachelor’s degree in nursing.
  • Required: 5 years of clinical nursing experience.
  • Preferred: 1 year of palliative care or hospice experience.
Certifications
  • Required: Current RN license in the state of practice.
  • Required: BLS certification from the American Heart Association.
  • Required: Hospice Palliative Care Nursing certification (to be obtained within 2 years of hire).
Knowledge, Skills, and Abilities
  • Proficiency with computers and software applications.
  • Effective communication skills, both verbal and written, tailored to diverse audiences.
  • Flexibility to work varied schedules, including weekends, holidays, on-call, and travel between facilities.
Job Duties
  • Ensure continuity of care across settings.
  • Assess and update care plans for palliative care patients.
  • Coordinate referrals respecting ethical principles and patient preferences.
  • Utilize resources to enhance patient and family quality of life.
  • Build relationships with patients and multidisciplinary teams for seamless care and follow-up.
  • Educate patients and families about palliative care considering individual characteristics.
  • Provide empathetic support and outreach throughout care plans.
  • Adapt communication and behavior to suit patient populations, respecting privacy and obtaining permissions.
  • Collaborate with physicians and the healthcare team to coordinate and communicate care plans, advocating for patients.
  • Perform other related duties as assigned.
