This role acts as a liaison and communicator with patients, caregivers, healthcare providers, and the multidisciplinary team, including post-acute care and third-party payers. The nurse assesses and addresses the needs of patients with advanced disease or at the end of life, providing educational support to patients, families, and healthcare professionals, including physicians and fellow nurses, in a positive and empathetic manner.
