This role functions as a liaison and communicator with patients, caregivers, healthcare providers, and multidisciplinary team members, including post-acute care and third-party payers. The nurse assesses and meets the needs of patients with advanced disease and at end of life, providing positive educational experiences to patients, families, and healthcare professionals, including physicians and nurses. The nurse interacts with patients and families empathetically.

Minimum Requirements
  • Graduate of an accredited nursing school
  • Current RN license in the state of practice
  • Basic Life Support (BLS) certification from the American Heart Association
  • 5 years of clinical nursing experience
Preferred Qualifications
  • Bachelor’s degree in nursing
  • 1 year of palliative care nursing or hospice experience
  • Hospice Palliative Care Nursing certification (must obtain within 2 years of hire)
Knowledge, Skills, and Abilities
  • Proficiency with computers and web-based applications
  • Effective verbal and written communication skills
  • Ability to work flexible hours, including weekends, holidays, and on-call, and travel between facilities
Job Duties
  • Ensure continuity of care across settings
  • Assess and modify care plans for palliative patients
  • Coordinate referrals, advocating for patient and family wishes
  • Utilize resources to enhance patient and family quality of life
  • Build relationships with patients and multidisciplinary teams for ongoing care
  • Educate patients and families about palliative care, respecting personal characteristics
  • Provide empathetic counseling and outreach
  • Adapt communication style to patient populations
  • Coordinate with physicians and team members to maintain updated, patient-centered care plans
  • Perform other related duties as required
