This job functions as the liaison and communicator with patients, caregivers, healthcare providers, and multi-disciplinary team members, as well as post-acute care and third-party payers. It involves participating in the interdisciplinary team to assess and meet the needs of patients and their families with advanced disease and those at the end of life. The role includes providing positive educational experiences to patients, families, and healthcare professionals, including physicians and nurses, and interacting with patients and families in a positive, empathetic manner.

Required: Graduate of an accredited school of nursing.
Preferred: Bachelor’s degree in nursing.
Required: 5 years of clinical nursing experience.
Preferred: 1 year of palliative care nursing or hospice experience.
Required: Current RN license in the state of practice, Basic Life Support (BLS) from the American Heart Association, Hospice Palliative Care Nursing certification (must obtain within 2 years of hire).
