This job functions as the liaison and communicator with patients, caregivers, healthcare providers, and multi-disciplinary team members, as well as post-acute care and third-party payers. It involves participating in the interdisciplinary team to assess and meet the needs of patients and their families with advanced disease and those at the end of life. The role includes providing positive educational experiences to patients, families, and healthcare professionals, including physicians and nurses, and interacting with patients and families in a positive, empathetic manner.

Education

Required: Graduate of an accredited school of nursing.

Preferred: Bachelor’s degree in nursing.

Work Experience

Required: 5 years of clinical nursing experience.

Preferred: 1 year of palliative care nursing or hospice experience.

Certifications

Required: Current RN license in the state of practice, Basic Life Support (BLS) from the American Heart Association, Hospice Palliative Care Nursing certification (must obtain within 2 years of hire).

Knowledge, Skills, and Abilities

• Proficiency with computers, software, and web applications.
• Effective verbal and written communication skills to present information clearly to various audiences.
• Ability to work flexible hours, including weekends, holidays, on-call, and travel between facilities.

Job Duties

• Ensure continuity of care through smooth transitions between settings.
• Assess and modify care plans for palliative patients.
• Coordinate referrals, respecting patient and family wishes, and advocate for their preferences.
• Utilize resources to improve patient and family quality of life.
• Build relationships with patients and multidisciplinary teams for ongoing care and follow-up.
• Educate patients and families about palliative care, considering individual characteristics.
• Provide empathetic support and outreach.
• Adapt communication and behavior to the patient population, respecting privacy and preferences.
• Work closely with physicians and the healthcare team to coordinate care and advocate for the patient.
• Perform other duties as required, ensuring compliance with laws and standards.
