This role functions as a liaison and communicator with patients, caregivers, healthcare providers, and multidisciplinary team members, including post-acute care and third-party payers. The nurse assesses and meets the needs of patients with advanced disease and those at the end of life, providing positive educational experiences to patients, families, and healthcare professionals, including physicians and fellow nurses. Interactions with patients and families should be conducted in a positive, empathetic manner.
