This role functions as a liaison and communicator with patients, caregivers, healthcare providers, and multidisciplinary team members, including post-acute care and third-party payers. The nurse assesses and meets the needs of patients with advanced disease and those at the end of life, providing positive educational experiences to patients, families, and healthcare professionals, including physicians and fellow nurses. Interactions with patients and families should be conducted in a positive, empathetic manner.

Education
  • Required: Graduate of an accredited school of nursing.
  • Preferred: Bachelor’s degree in nursing.
Work Experience
  • Required: 5 years of clinical nursing experience.
  • Preferred: 1 year of palliative care nursing or hospice experience.
Certifications
  • Required: Current RN license in the state of practice.
  • Required: Basic Life Support (BLS) from the American Heart Association.
  • Must obtain Hospice Palliative Care Nursing certification within 2 years of hire.
Knowledge, Skills, and Abilities
  • Proficiency in computer and web-based applications.
  • Effective verbal and written communication skills, with the ability to present information clearly and professionally.
  • Flexible work schedule, including availability for weekends, holidays, on-call, and travel between facilities.
Job Duties
  • Ensure continuity of care through smooth transitions between settings.
  • Assess and modify care plans for palliative care patients as needed.
  • Coordinate referral processes, advocating for patient and family wishes, respecting ethical principles and professional standards.
  • Use resources effectively to enhance patient and family quality of life.
  • Build relationships with patients and multidisciplinary teams to facilitate treatment and follow-up.
  • Educate patients and families about palliative care, considering personal characteristics.
  • Provide empathetic support and outreach throughout the care plan.
  • Adapt communication style to respect privacy and patient preferences.
  • Collaborate with physicians and the healthcare team to coordinate and communicate care plans, advocating for the patient.
  • Perform other related duties as required.

Physical and Environmental Demands

This role involves medium work, exerting 20-50 pounds of force occasionally, and/or 10-25 pounds frequently, with exposure to blood, body fluids, and tissues. The employee works in patient care areas with potential occupational risks for exposure to communicable diseases and hazardous materials, requiring adherence to safety protocols.
