Qualifications

• RN license in Illinois (state) CPR certification.

• Board Certification within two years of hire or by 12/31/2015.

Responsibilities

• Assess and coordinate care for oncology patients.
• Facilitate patient education on treatment options and readiness.
• Coordinate clinic visits and procedures with physicians and staff.
• Support continuity of care across day surgery, inpatient, and outpatient units.
• Utilize EMR tools to track tests, results and referrals.
