Job summary

Fairview Range Hibbing Clinic seeks an experienced Registered Nurse II in Oncology Care Coordination. You will assess patients, coordinate care across the treatment continuum, and collaborate with an interdisciplinary team to optimize outcomes.

The role includes IV/VAD maintenance and chemotherapy administration in an outpatient setting, with a focus on education and cost-conscious care. The position is full-time, with 72–80 hours every two weeks, involving day/evening shifts and some weekends

Qualifications

• RN with oncology-focused care coordination experience required
• License to practice as a Registered Nurse in Minnesota
• Oncology chemo/immuno certification within required timelines
• BSN preferred but not required depending on candidate

Responsibilities

• Assess patient and family needs and plan care across episodes or continuum
• Collaborate with interdisciplinary team to develop clinical pathways
• Coordinate care plans with physicians and other departments
• Administer IV/VAD medications and chemotherapy as ordered
• Educate patients and families about treatment options and resources
• Plan short- and long-term care goals with clear time frames
• Communicate care plans to payers to support appropriate reimbursement
