American Senior Communities, LLC is seeking a Registered Nurse (RN) for a resident care coordination role with a management path. The position supports continuity of care across shifts, including evening admissions and weekend coverage.

Expect to lead care planning, educate direct caregivers, and uphold CARE values in every interaction. We value compassionate care, accountability, relationships, and excellence in our community-focused nursing environment.

Qualifications

• Graduated from an accredited school of nursing and hold a current Indiana RN license.
• Minimum of three years' clinical experience in geriatric or gerontology with at least one year as a Departmental Manager of a Long-Term Care setting.
• Prior consulting experience is preferred.

Responsibilities

• Coordinate resident care between shifts to ensure continuity and communicate changes.
• Monitor new admissions to ensure appropriate care plan interventions are in place.
• Ensure evening admissions transition smoothly into the health care community with resident needs met.
• Coordinate evening/weekend discharges to ensure proper interventions are in place.
• Provide education and training to direct care givers on changes in condition.
• Monitor residents with changes in condition or recent falls for continuity of care.
• Assist the Director of Nursing Services with daily clinical functions and DNS-related tasks.
• Demonstrate CARE values in interactions with residents, families, staff, and colleagues.
• Comply with HIPAA and privacy laws for resident and employee records.
