Job summary

American Senior Communities is seeking a Registered Nurse for East Lake Nursing & Rehabilitation Center in Elkhart, IN. The role focuses on coordinating resident care across shifts, monitoring admissions, and ensuring effective care plans.

The position includes an on-call rotation and a 10am-6pm schedule with flexible hours under a compassionate CARE framework. Responsibilities include supervising care delivery, educating direct care staff, and collaborating with the Director of Nursing Services

Qualifications

• Graduated from an accredited school of nursing and hold a current Indiana Registered Nurse license.
• Experience in geriatric care or long-term care management preferred.
• Strong communication and leadership abilities required.

Responsibilities

• Coordinate resident care between shifts to ensure continuity of care.
• Monitor admissions and ensure care plans are in place across shifts.
• Oversee evening/weekend discharge planning to maintain quality care.
• Provide education and training to direct care staff on changes in condition.
• Support the DNS with daily clinical operations and accident/inquiry investigations.
• Demonstrate CARE values in interactions with residents, families, and staff.
