American Senior Communities, LLC is seeking a Resident Care Coordinator – Registered Nurse (RN) to ensure seamless care transitions across shifts and to oversee admissions and discharges with a focus on resident well-being. The role includes a $10,000 retention bonus and a 10am-6pm schedule with on-call rotation, reflecting our CARE values in daily practice.

Ideal candidates bring a valid Indiana RN license and at least three years of geriatric clinical experience, including one year in a LTCA

Qualifications

• Graduated from an accredited school of nursing and hold a current Indiana RN license.
• Minimum of three years clinical experience in Geriatrics; at least one year as a Departmental Manager in Long-Term Care.
• Prior consulting experience preferred.

Responsibilities

• Coordinate resident care between shifts to ensure continuity of care.
• Monitor new admissions to ensure appropriate care plan interventions.
• Oversee evening/weekend discharges to ensure proper care and safety.
• Provide education and training to direct care givers on changes in resident condition.
