Job summary

Rocky Mountain Health Care Services seeks an RN Home Care Coordinator to integrate with the IDT, present nursing interventions for care planning, and report changes in condition for participants. You will manage orders, provide phone triage, and schedule home care nursing.

The role requires Colorado RN licensure, with prior hospital/long-term/home health experience preferred, and strong communication and documentation skills. Comprehensive benefits are offered.

Qualifications

  • Graduate of an accredited Registered Nurse Program and Colorado license.
  • Minimum of one year healthcare experience preferred.
  • CPR and First Aid certification preferred.
  • Excellent written and verbal communication with internal and external customers.
  • Proficient in Microsoft Office and EMR systems.
  • Knowledge of medical equipment and nursing procedures.
  • Ability to apply nursing principles for ongoing participant care.
  • Commitment to department quality assurance.

Responsibilities

  • Attends IDT meetings and presents nursing interventions for care planning.
  • Performs chart audits and ensures nursing documentation is complete and accurate.
  • Supports home care nurses with procedures, documentation, and task organization.
  • Performs home visits to administer medications and treatments per nursing standards.
  • Covers after-hours and weekend on-call rotation with RMHCS NPs and RNs as needed.
  • Documents interactions in the EMR and reports participant condition to physicians.
  • Instructs participants and family on medications and treatment instructions.
  • Maintains supplies for home visits and ensures cleanliness.
  • Keeps IDT informed about home care issues and educates on plans of care.

Salary Range: $38.38 - $42.22 Hourly

Additional per diem is also offered on top of the pay scale listed.

POSITION SUMMARY:

The RN Home Care Coordinator position serves as a member of the Interdisciplinary Team (IDT) under the CMS regulations. As such, they are responsible for attending IDT meetings to present nursing interventions for care planning and to report on changes in condition for Rocky Mountain Health Care Services (RMHCS) participants. Participates in the communication of the participants condition to the RN Home Care Manager and the IDT. This position manages provider orders, provides phone triage to participants, and manages the schedule for home care nursing.

RESPONSIBILITIES AND DUTIES:
ESSENTIAL JOB FUNCTIONS:
  • Serves as a member of the IDT
  • Presents nursing interventions for participant care planning, fall reports, and incident reports
  • Performs chart audits and ensures that nursing documentation is complete and accurate
  • Acts as a support to home care nurses regarding procedures, documentation, and task organization
  • Performs home visits to administer prescribed medications and treatments in accordance with nursing standards to supplement clinic services as needed
  • Covers after hour and weekend call in rotation with other RMHCS NPs and RNs as needed.
  • Observes, records, and reports participants condition and reaction to drugs and treatments to physicians
  • Documents all interactions related to home care visits in the EMR
  • Instructs participants and family members regarding medications and treatment instructions
  • Keeps Home Care Manager and IDT informed on home care issues of participants as they arise
  • Posts advance directives in participants homes and educate participants and families regarding plans of care
  • Attends IDT meetings at care conferences, family meetings, and any other meetings as assigned
  • Maintains personal supplies and materials needed for home visits. Ensures cleanliness. Prepares list of medical supplies needed and in stock
  • Participates in continuing education classes and any required staff and training meetings. Maintains professional affiliations and any required certifications
  • Other duties as requested/assigned
Qualifications
  • Graduate of an accredited Registered Nurse Program and licensed as a professional in the State of Colorado is required
  • Minimum of one year of health care experience in hospital, long-term or home health care setting is preferred
  • CPR and First Aid certification is preferred
  • Must have excellent written and verbal communication skills and demonstrate the ability to interact clearly and effectively with both internal and external customers
  • Proficient in software programs, such as Microsoft office and electronic medical records
  • Knowledge of medical equipment and instruments
  • Skill in applying and modifying the principles, methods and techniques of practical nursing to provide on-going care for participants
  • Skilled in developing and maintaining department quality assurance.
