Job summary

Rocky Mountain PACE in Colorado Springs is seeking an experienced Registered Nurse to provide skilled nursing assessments and work with the Interdisciplinary Team to meet care plan goals.

You will communicate with families, educate patients, and participate in comprehensive care coordination while delivering high-quality nursing care in a team-focused environment.

Qualifications

• Graduate of an accredited Registered Nurse Program and licensed in Colorado.
• Healthcare experience in hospital, long-term or home health care; 1 year with frail/elderly preferred.
• Reliable transportation and current auto insurance required.
• Competent with computer applications including EHR, email and internet.
• Knowledge of medical equipment and instruments.

This position is responsible for working directly with the Interdisciplinary Team (IDT), participant, and his or her representative to achieve the goals of the care plan.
