Job summary

White Oak of Charlotte is seeking a Registered Nurse to serve as MDS Coordinator/Nurse Assessment Coordinator. In this leadership role, you will oversee the Resident Assessment Instrument (RAI) process, MDS assessments, CAAs, and individualized care planning in line with federal and state regulations.

You will collaborate with nursing, therapy, physicians, and the interdisciplinary team to ensure high-quality, person-centered care while maintaining compliance and reimbursement accuracy.

Qualifications

• Active SC RN license.
• Minimum 1 year long-term care experience.
• MDS Coordinator experience preferred.
• Knowledge of Medicare PPS, PDPM, and Managed Care.

Responsibilities

• Coordinate interdisciplinary resident assessment and care planning.
• Complete MDS assessments (admission, readmission, quarterly, annual, significant change, discharge).
• Complete CAAs and personalized care plans within regulatory timelines.
• Lead care plan meetings and family conferences.
• Communicate updates to families, physicians, and team.
• Manage Medicare PPS, PDPM, and Managed Care schedules.
• Ensure documentation supports accuracy and reimbursement.
