A healthcare provider in Elizabeth City, North Carolina is looking for a motivated Registered Nurse (RN) Minimum Data Set (MDS) Coordinator to join their compassionate team. Qualifications

• Current license as a Registered Nurse issued by the State Board of Nursing.
• Experience with the RAI process.
• Knowledge of PDPM and State Case Mix Time weighted Reimbursement payment system.
• Knowledge of long-term care laws/regulations.
• Strong English communication skills, both verbal and written.
• Customer service oriented with ability to work under pressure.
• Excellent organizational skills and attention to detail.

Responsibilities

• Coordinates the completion of the MDS by all disciplines.
• Maintains schedule of due dates for all MDSs.
• Formulates Resident Care Plan along with interdisciplinary care plan team.
• Completes the MDS and inputs data into the computer.
• Verifies assessments transmitted and approved by the State.
• Corrects rejected records and prepares for re-submission.
• Formulates Resident Care Plan with interdisciplinary team.
• Schedules and conducts Care Plan meetings regularly.
• Communicates Care Plan to staff and notifies supervisor of noncompliance.
• Works with all departments to ensure Care Plan understanding and compliance.
• Performs other related duties as directed by the Director of Nursing Services and Administrator.

Job description

Elizabeth City Health and Rehab is looking for a motivated Registered Nurse (RN) Minimum Data Set (MDS) Coordinator to join our compassionate team of caregivers. The MDS Coordinator completes assessments and care plans for residents using the Resident Assessment Instrument (RAI) process in accordance with the laws, regulation, and company standards. The MDS Coordinator reviews and coordinates Patient Driven Payment Model (PDPM) and Case Mix data for the facility to ensure appropriate reimbursement. This position is for an RN and is Full Time.

Job Requirements

• Communicates the Care Plan to appropriate staff members and notifies supervisor if noncompliance by staff is noted.
• Works with all in-house and ancillary departments to assure understanding and compliance with the Care Plan.
• Performs other related duties as directed by the Director of Nursing Services and Administrator.
