Job summary

White Oak of Charlotte, part of NATIONAL HEALTH - WHITE OAK LLC, seeks an experienced MDS Coordinator/Nurse Assessment Coordinator (RN) to lead the RAI process in a resident-centered long-term care setting.

The role requires an active RN license, at least 1 year in long-term care, and familiarity with MDS, CAAs, and regulatory compliance. You will collaborate with an interdisciplinary team to ensure accurate assessments and optimal reimbursement.

Qualifications

• Active South Carolina RN license
• Graduate of an accredited school of nursing
• Minimum one year of long-term care experience
• MDS Coordinator experience preferred
• Working knowledge of Medicare, PPS, PDPM, and Managed Care reimbursement
• Strong understanding of federal and state long-term care regulations
• Excellent assessment, organizational, communication, and leadership skills
• Experience with electronic medical record (EMR) systems

Responsibilities

• Coordinate the interdisciplinary resident assessment and care planning process
• Complete accurate and timely MDS assessments (admission, readmission, quarterly, annual, significant change, and discharge)
• Complete Care Area Assessments (CAAs) and individualized care plans within required regulatory timelines
• Review and update care plans to reflect residents' changing needs and goals
• Lead and participate in care plan meetings and family conferences
• Communicate resident care plan updates with families, physicians, and the interdisciplinary team
• Manage Medicare PPS, PDPM, and Managed Care assessment schedules
• Ensure clinical documentation supports assessment accuracy and reimbursement requirements
• Assist with state and federal surveys and maintain regulatory compliance
• Stay current on MDS regulations, reimbursement guidelines, and industry best practices
• Educate staff on documentation and resident assessment processes
• Maintain accurate resident records using electronic medical record (EMR) systems
• Perform additional duties as assigned by the Director of Nursing
